1. NO Slack on the phone. I am deleting the app for my week off
2. Emegrency communication channel is email. There is never anything urgent enough that can not wait till I check the mailbox.
3. No checking metrics! no analytics, no social media, no crm.
4. Don't hang out with co-workers, or they will drag you back into the work brain mode

I cannot really agree with any of your rules, they simply do not apply in my case :) But it's good you found some ways of building your work-life balance!
I have plenty of rules that I try to follow and they help me keep my brain and body relaxed during time off work :D
One of the most important ones is to have all social media notifications turned off and set up a range of contacts with high priority that could contact me anytime they want - for the rest, I can always call them back later! :)
