Keep it simple - 1. General health, issues, any day to day challenges which you can address, Needs, Wants 2. Risk and Concerns related to work. 3. Any guidance/ mentorship/ assistance. 4. Feedback, and action item/ goals/ milestones/ plan. Have a expected action in the agenda. Try to make life easier in 1:1.
