I like your design. I would start using it if it could do more than storing a birthdate. I actually think you have a good basis for an app I wanted to create: something that suggests presents. You could integrate a list of suggested presents based on the age of the owner. And allow users to trigger the order via your app (earn affiliate fee)

Web UI to create the quiz looks super clean. Good work. The App screenshots look a bit too noisy. Maybe less color (or just a simple gradient) would improve the overall appeal. Don't have a use case it atm but would consider using it if I had..
